Your support plan

Your social worker will have already created a Pathway Plan that highlights the goals you hope to achieve during your stay with us. This plan serves as an essential foundation for us to support you effectively.

Personalised support

To help you reach your goals, you and your support worker will put together a personalised support plan of what you want to achieve and how you want to achieve it while you are living here.

Your plan might include things like:

  • enrolling at college or university
  • getting involved in the community
  • seeking support with mental health challenges
  • opening a bank account and managing your finances
  • registering with a doctor or dentist
  • access to healthcare and sexual health services
  • education and employment opportunities.

The support plan outlines what lies ahead and the steps we need to take together to help you achieve your goals and what support, if any, you might need.

You will be involved in all aspects of your support plan including setting your own goals. Your thoughts, opinions and wishes matter and we want you to feel comfortable and empowered.

Your plans are private and we will keep them secure.

You will meet with your support worker privately at least once a week to discuss your support plan. We are here to guide, assist and celebrate your progress.

As well as a support plan we will also create an activity plan with you. This plan will help you to organise your schedule and ensure that you have a healthy balance between your commitments, personal goals and things you enjoy doing. In this plan you will include things like your education or work schedule, any upcoming appointments and other responsibilities you might have.

You will also have a personalised Strengths Assessment. This will address any specific issues and challenges you may be facing and how we can help you to stay safe and well.

We will keep a record of your time with us using an Independence Tracker. This document captures important details about your activities, accomplishments and attendance in employment, training and/or education. At the end of each month, you will have the opportunity to review your tracker with your support worker.
